When you submit our transfer-request form, we collect only basic contact details: your name, phone number, optional email address, and preferred callback time. The form does not ask for, and should never be used to send, medication names, prescription numbers, dates of birth, or any other health information; a pharmacist collects those details with you by phone. We use your contact details solely to respond to your request.
Your health information
Please do not include sensitive medical details (such as diagnoses or specific medication lists) in the website form. The form is for basic contact and to start a prescription transfer; a pharmacist will follow up with you directly to handle anything involving your health information. The protected health information in your pharmacy records is handled under applicable federal and Illinois law, including HIPAA, and is governed by our in-store Notice of Privacy Practices, available at the pharmacy on request.
Analytics
We use Google Analytics to understand how visitors use our site (pages visited, time on site, general location at city level). This data is aggregated and does not personally identify you. You can opt out with the Google Analytics opt-out browser add-on.
Cookies
This site uses cookies only for analytics as described above. We do not use advertising cookies or sell data to third parties.
How we share information
